Be ForeverFit

Exercise and nutrition programs can help you be healthier and independent. Advantra® ForeverFit provides membership to any participating gym or health club at no extra cost and with no initiation fees. 

You also get special discounts on a wide variety of vitamins and nutritional supplements, as well as select alternative health services like acupuncture, massage therapy and nutrition counseling. Plus, ForeverFit includes SeniorPhit, an interactive online wellness program that provides personalized planning and coaching for exercise, diet and self-improvement.

Falls & Fracture Prevention

Falls are one of the leading causes of injuries and hospital admissions. To properly evaluate your true risk, you need the right assessment tools and information—along with tips to help you avoid falls. We think it’s so important that we’ve created a program to help you do just that.

Disease Management Programs

Coventry Health Care has developed and implemented a program to  provide comprehensive care management to members with congestive heart failure (CHF) and diabetes. The program provides ongoing, comprehensive care that increases the member’s awareness of his or her condition and the value of its treatment and self-management. With proper education and self-care, people with CHF and diabetes can prevent or minimize related complications and have healthier lives.

If you are new to Advantra (HMO-POS) and have not already filled out a Transition of Care form, click here to print a copy. Fill out the information and follow the instructions for returning this form to Coventry.

Dental, Hearing and Vision Benefits

Some of our plans include coverage to help you take care of your teeth, your hearing and your sight. Benefit options vary by plan, but may include routine check-ups as well as coverage for hearing aids and glasses. Please visit My Online Services, call us at the toll-free number on your member ID card, or review your Evidence of Coverage to determine which options are available through your particular plan.

Provider Search

Coventry’s provider search function is updated regularly. While it is our goal to provide accurate information, the composition of our provider network is subject to change without notice. The participation of a particular provider is never guaranteed, even if a member is referred by their physician to the provider. Coventry Health Care continually negotiates with additional physicians and hospitals, and changes to our network list are made as appropriate. It is important to know when you enroll in this plan, services are provided through the plan's delivery system, but continued participation of any one doctor, hospital, or other provider cannot be guaranteed.

Members should always verify that the provider they choose is participating in our network before they receive services.
